Job summary

Suncrest Home Health, part of the LHC Group within the Optum family, seeks a Per Diem Physical Therapist Assistant to deliver PT services in home care, following the plan of care set by a supervising Physical Therapist. The role emphasizes patient education, exercise instruction, and coordinating with the care team.

Responsibilities include teaching active and passive exercises, gait training, ADLs, and safe transfers, while ensuring compliance with state laws and facility policies.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.
• Preferred CPR certification or ability to complete within 90 days of hire.
• Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility.

Responsibilities

• Following all state laws governing the provision of physical therapy in home care and adhering to the plan of care defined by the supervising PT.
• Instructs and aids patients in active and passive exercise, gait, ADL, transfer, safety, and prosthetic training.
• Observes, records, and reports patient response to treatment to the supervising PT and coordinates care with the health care team.
