Suncrest Home Health, part of the LHC Group and Optum family, is hiring a Per Diem Physical Therapist Assistant to support patients in their homes under the supervision of a Physical Therapist.

You will provide treatment, teach exercises, and educate patients and families to improve well-being while following the established plan of care. The role offers flexible per diem scheduling and collaboration with a multidisciplinary team.

Formación

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsabilidades

• Follow all state laws governing the provision of physical therapy in home care and the plan of care.
• Instruct and aid patients in active and passive exercises, gait, ADL, transfers, and prosthetic training.
• Observe and report patient response to treatment to the supervising PT and care team.
• Educate patient, family, and others in therapy within the scope of physical therapy assistant.
• Function in diverse home situations and contribute to care coordination.
