Suncrest Home Health, part of the LHC Group and Optum family, seeks a Per Diem Physical Therapist Assistant to deliver treatment and patient education under a supervising PT in home health settings.

The role includes assisting with active and passive exercises, gait training, and functional activities; documenting patient responses and coordinating with the healthcare team. Weekend rotation is required, with a competitive pay structure and benefits.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.

Responsabilités

• Follow state laws and plan of care defined by supervising PT.
• Instructs and aids patients in active and passive exercise, gait, ADL, transfer, and safety.
• Observe, record, and report patient responses; coordinate care with the health team.
• Instructs patient, family, and other care team members in therapy within PT assistant scope.
• Function in diverse home settings regardless of client demographics.
