Suncrest Home Health, part of the LHC Group and affiliated with Optum, seeks a Per Diem Physical Therapist Assistant to support home health patients under a supervising PT. The role emphasizes exercise instruction, patient education, and contributing to the plan of care.

Responsibilities include performing treatments, documenting progress, and coordinating with nurses and physicians while ensuring high-quality patient care in diverse home settings. Weekend availability is required.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver’s license and vehicle insurance or access to dependable transport.
• Available to work a weekend rotation.

Responsabilités

• Follow state laws and the supervising PT’s plan of care.
• Instruct patients in active and passive exercises and gait training.
• Record and report patient responses to treatment to supervising PT and care team.
• Instruct patients, families, and caregivers in therapy techniques.
• Maintain ability to function in diverse home environments.
