Job summary

Suncrest Home Health, part of the Optum family, is seeking a Per Diem Physical Therapist Assistant to perform treatments, provide exercise instruction, and patient education under the plan of care in home health settings.

Responsibilities include following state laws, assisting with exercise, recording patient responses, coordinating with the healthcare team, and educating patients and families. Weekend rotation availability and valid PTA license required.

Qualifications

• Licensed or certified to assist in physical therapy in the practicing state.
• Ability to travel to patient homes and access to transport.
• Willingness to work a weekend rotation.

Responsibilities

• Follow supervising PT's plan of care and state laws.
• Instructs patients in active and passive exercise, gait, ADLs.
• Observe and report patient responses to treatment to PT and nurse/physician.
• Educate patient, family, and care team about therapy within scope.
• Adapt to home environments and diverse patient needs.
