Job summary

Responsibilities include guiding patients through therapeutic exercises, coordinating with the healthcare team, and ensuring adherence to state practice laws. Weekend rotation required; CPR or timely certification desired.

Qualifications

• Current, licensed to assist in the practice of physical therapy in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.
• CPR certification or ability to complete within 90 days of hire.
• Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility.

Responsibilities

• Follow state laws governing physical therapy in home care and work under supervising PT.
• Instructs patients in active and passive exercise, gait, ADLs, and safety.
• Observes and reports patient responses to treatment to the supervising PT and care team.
• Educates patient, family, and care team on therapy scope and plan of care.
• Function in diverse home settings regardless of background.
