Job summary

UnitedHealth Group is seeking a Physical Therapist for home health services to evaluate and treat patients, develop and implement treatment plans, and monitor progress. The role involves working with physicians and caregivers to improve mobility, endurance, and comfort in a home health setting.

Responsibilities include planning care, providing treatment, documenting outcomes, and ensuring adherence to state guidelines.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire (preferred)

Responsibilities

• Evaluates patient’s functional status and PT needs to help develop the plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient response to treatment to supervisor or physician.
• Treats patients to relieve pain and restore function.
• Instructs patient and family on plan of care and progress toward goals.
• Documents visits and progress notes.
• Completes PT plan of care within state guidelines.
• Reports outcomes and frequency of care.
