Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health to assess patients, determine a treatment plan, and provide interventions aimed at improving mobility and overall well-being. You will collaborate with physicians and the care team, document progress, and ensure care adheres to state regulations and plan of care.

Responsibilities include performing evaluations, delivering PT services, educating patients and families, and visiting patients according to the treatment plan.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance or access to dependable transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates the patient’s functional status and PT needs to inform the plan of care.
• Provides PT services within scope of practice and plan of care with the care team.
• Observes patient responses and reports changes to nurse or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Educates patient and family about plan of care, limitations, and progress.
• Visits patients per treatment plan and documents progress notes.
