Qualifications

• Graduate of a CAPTE-accredited physical therapy program.
• Current, unencumbered license to practice physical therapy in the assigned state.
• Driver's License and access to reliable transportation.
• Current CPR certification.
• 1+ years of experience in physical therapy; OR less than one (1) year with participation in an approved residency or mentorship program.
• 1+ years of experience in clinical assessment, documentation, and care coordination.

Responsabilités

• Conduct comprehensive patient assessments and develop individualized PT plans of care in collaboration with physicians.
• Provide skilled PT interventions and modify treatment plans based on progress and clinical needs.
• Perform ongoing reassessments, including weekly evaluations, to support care planning and physician coordination.
• Collaborate with interdisciplinary teams to coordinate care and communicate patient progress and condition changes.
• Maintain accurate, timely documentation in compliance with clinical, regulatory, and documentation standards.
