UnitedHealth Group is seeking a Physical Therapist for home health to evaluate patients, develop treatment plans, and coordinate care with the health team. You will treat pain, restore function, and educate patients and families in the home setting.

Responsibilities include conducting assessments, documenting progress, and ensuring care aligns with state guidelines and plan of treatment. A supportive team and comprehensive benefits are offered.

Formación

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsabilidades

• Evaluates the patient's functional status and physical therapy needs; assists in development of the plan of care.
• Provides services within the scope of practice as defined by state laws; coordinates with health care team.
• Observes and reports patient response to treatment and changes in condition to supervising nurse/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding plan of care, limitations, and progress toward goals.
• Visits patients according to Plan of Treatment; completes progress notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
