Amedisys, part of the Optum family, seeks a Physical Therapist to help patients restore function and mobility at home under a physician-directed plan of care, coordinating with the interdisciplinary team.

Responsibilities include assessments, treatment planning, interventions, and timely documentation. Requirements: CAPTE-accredited PT program, current state license, CPR, and 1+ year of PT experience. Salary ranges from 80,864 to 121,296 USD annually.

Competenze

• Graduate of an accredited physical therapy program (CAPTE accredited).
• Current, unencumbered license to practice physical therapy in the assigned state.
• Current CPR certification.
• 1+ years of experience in physical therapy; OR less than one year with residency/mentorship.
• 1+ years of experience in clinical assessment, documentation, and care coordination.

Mansioni

• Conduct comprehensive patient assessments and develop individualized PT plans of care in collaboration with physicians.
• Provide skilled PT interventions and modify plans based on progress and needs.
• Perform ongoing reassessments to support care planning and physician coordination.
• Collaborate with interdisciplinary teams to coordinate care and communicate progress.
• Maintain accurate documentation in line with clinical, regulatory, and documentation standards.
