Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance or public transportation access.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs and assists the physician in developing the plan of care.
• Provides services within the scope of practice and plan of care, coordinating with the health care team.
• Observes and reports patient response to treatment and any condition changes to the supervising nurse or physician.
• Treats patients to relieve pain and restore or maintain function according to standards of practice.
• Educates patient and family about plan of care, limitations, and progress toward goals.
• Visits patients per Plan of Treatment, completes progress notes, and submits per policy.
• Completes all evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
