• Current Physical Therapy licensure in state of practice.
• Current CPR certification.

Responsibilities

• Evaluates patient's functional status and assists in therapy plan of care.
• Provides PT services within scope of practice as per plan of care.
• Observes and reports patient responses to treatment to supervisor/physician.
• Treats patients to relieve pain and restore function.
• Educates patient and family about plan of care and progress toward goals.
• Visits patients per Plan of Treatment and documents progress notes.
• Develops PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
