Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation
• Preferred CPR certification (within 90 days of hire)

Responsibilities

• Evaluates the patient’s functional status and physical therapy needs.
• Assists physician in development of the plan of care.
• Provides services within scope of practice and in coordination with health care team
• Observes and reports patient responses to treatment and changes in condition
• Treats patients to relieve pain and restore function
• Instructs patient and family on plan of care and progress toward goals
• Completes patient evaluations and PT plan of care within state guidelines
• Reports outcomes of evaluation and projected frequency of care
