Qualifications

• Current Physical Therapy licensure in state of practice.
• Driver's license and access to a dependable vehicle or reliable transportation.

Responsibilities

• Evaluates the patient's functional status and therapy needs.
• Assists in development of the plan of care with physician.
• Provides PT services within state scope of practice and plan of care.
• Observes and reports patient responses and changes in condition.
• Implements interventions to improve mobility and function.
• Educates patient and family about plan of care and progress.
• Completes progress notes and evaluates outcomes.
• Works with the care team to coordinate treatment.
