Job summary

Caretenders, part of LHC Group and a member of the Optum family, is seeking a Physical Therapist for Home Health. You will assess patients, develop treatment plans, and deliver interventions to improve mobility and well-being in a home setting.

Responsibilities include evaluating functional status, coordinating with the health care team, and documenting progress. A state-licensure and driving routine are required, with CPR preferred.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding plan of care and progress toward goals.
• Completes progress notes and PT plan of care within state guidelines.
