Job summary

UnitedHealth Group, through Mederi Caretenders, seeks a Physical Therapist to assess and plan patient care in home health, focusing on mobility, endurance, pain, and well-being. You will develop and implement a treatment plan, deliver interventions, and monitor progress in patients' homes.

You will collaborate with healthcare teams, document visits, and ensure care aligns with state practice guidelines while supporting families. A competitive compensation package and benefits are included.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists the physician in the plan of care.
• Provides services within the scope of practice and in coordination with the health care team.
• Observes and reports the patient's response to treatment to the supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Informs patient and family regarding the plan of care, goals, and progress.
• Visits patients per Plan of Treatment; completes progress notes and submits per policy.
• Completes all evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes and frequency of care; adapt to patient needs.
• Function effectively in any home setting regardless of demographics.
