Caretenders, a part of LHC Group and within the Optum family, is seeking a Physical Therapist in Home Health to assess patient needs related to mobility and endurance, determine a treatment plan, and perform interventions aimed at improving well‑being.

You will evaluate progress, coordinate with the health care team, and document outcomes in accordance with state guidelines. A rewarding role with opportunities for growth.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsabilités

• Evaluates the patient's functional status and needs; assists with therapy plan of care.
• Provides services within scope of practice and in coordination with the health care team.
• Observes and reports patient responses to treatment and changes in condition.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers about plan of care, limitations, and progress.
• Visits patients per Plan of Treatment; completes progress notes and submits per policy.
• Documents all evaluations and develops PT plan of care per state guidelines.
