Job summary

At Home Healthcare, part of LHC Group and within the Optum family, seeks a Physical Therapist in Home Health to perform comprehensive assessments of functional status, mobility, endurance, and pain, forming a tailored treatment plan and measuring progress.

You will implement interventions to improve wellbeing, document visits, and communicate plans of care to patients and families, following state guidelines and policy.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists the physician in developing the therapy plan of care.
• Provides services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with other health care team members.
• Observes and reports the patient's response to treatment and any changes to the supervising nurse and/or the physician.
• Treats patients to relieve pain, develop and restore function, and/or maintain maximum functioning.
• Instructs and informs participating members of the health care team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
• Visits patients per Plan of Treatment, completes a progress note for each visit, and submits progress notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
• Able to function in home settings regardless of age, race, creed, color, sex, disability, or financial condition of the client.
