Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Develops and implements the therapy plan of care in coordination with the care team.
• Observes and reports patient responses to treatment to the supervising nurse/physician.
• Treats patients to relieve pain and restore function per standards of practice.
• Instructs patient and family about the plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes.
