Job summary

Amedisys, part of the Optum family, invites you to join our home health team as a Per Diem Physical Therapist in Florida. You will deliver skilled PT services in patients’ homes to promote mobility, function, and safety, collaborating with patients, caregivers, and the care team to support recovery and reduce hospitalizations.

Responsibilities include mobility improvement, 30-day reexaminations, discharge planning, and flexible scheduling.

Qualifications

• Graduate from a CAPTE-accredited physical therapy program.
• Current license to practice physical therapy in the state you're assigned to work in.
• 1+ year of home health PT experience; mentoring/residency if less than 1 year.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Conduct interventions, reexaminations (every 30 days, at minimum), modify as needed and perform discharge planning.
• Other duties as assigned.
