VitalCaring Group in Crystal River, FL is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes across the Crystal River branch service area.

You’ll work autonomously with nurses and physicians, evaluate function, develop plans, and educate patients and caregivers, with opportunities for ongoing clinical education and career growth. Join a compassionate team transforming lives through quality home health care.

Qualifikationen

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health preferred).
• Current CPR certification.

Aufgaben

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines.
