Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes across Crystal River and surrounding communities. You will assess functional status, develop individualized plans with physicians, and provide hands-on therapy to restore mobility and independence.

The role emphasizes autonomy, collaboration with the care team, and ongoing education for patients and families, with emphasis on safe transitions and timely documentation in the EMR.

Qualifications

• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Partner with nurses, therapists, and physicians to ensure coordinated care.
• Document patient care and progress accurately and timely within the EMR.
