Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver in-home care across Crystal River and surrounding communities in Florida. You’ll evaluate mobility, develop individualized plans, and work closely with physicians and caregivers to maximize independence.

Ideal candidates have a DPT or Master’s in Physical Therapy, Florida PT license, and 1–2 years of home health experience, plus CPR and a valid driver’s license.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice (Florida).
• Minimum 1–2 years of home health experience preferred.
• CPR certification and valid driver’s license required.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Supervise Physical Therapist Assistants (PTAs) as per state guidelines; coordinate with nurses and physicians.
