Zusammenfassung

VitalCaring Group is seeking a dedicated Home Health Physical Therapist to provide patient-centered care in patients’ homes across Crystal River and surrounding communities. This highly autonomous role involves evaluating mobility, developing tailored plans with the care team, and delivering hands-on therapy to improve function and independence.

You will educate patients and caregivers, monitor progress, and coordinate with nurses and physicians to ensure seamless transitions of care.

Qualifikationen

• Graduated from an accredited Physical Therapy program (DPT or Master's).
• Current PT license in the state of practice.
• 1–2 years of home health experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Aufgaben

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Partner with nurses, therapists, and physicians to ensure coordinated care.
• Document patient care and progress accurately and timely within the EMR.
• Participate in case conferences and team meetings.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines.
• Provide guidance and education to caregivers and support staff.
