VitalCaring Group in Crystal River, FL is seeking a Home Health Physical Therapist to evaluate, treat, and empower patients in their homes. This highly autonomous role emphasizes mobility, independence, and quality of life for patients we serve.

You’ll collaborate with nurses and physicians, educate families, and supervise PTAs as needed. We offer comprehensive benefits, flexible scheduling, and opportunities for professional growth.

Formación

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of home health experience (preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license and reliable transportation

Responsabilidades

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies
• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams
• Partner with nurses, therapists, and physicians to ensure coordinated care
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff
