Job summary

VitalCaring Group is seeking a dedicated Home Health Physical Therapist to deliver compassionate, patient-centered care in patients' homes across the Crystal River branch service area in Florida. You will assess mobility, design individualized plans with physicians, and guide patients toward independence in familiar surroundings.

This highly autonomous role demands 1–2 years of home health experience, an active PT license, CPR certification, and a valid driver’s license.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master's).
• Current PT license in the practicing state.
• 1–2 years of home health experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• CPR certification current.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehab potential.
• Develop and implement individualized home health care plans with the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Document care and progress accurately within the EMR.
• Collaborate with nurses, therapists, and physicians to ensure coordinated care.
• Supervise PTAs in accordance with state guidelines.
• Participate in case conferences and team meetings.
