Job summary

Residential Home Health and Residential Hospice in Traverse City, MI is seeking a Home Health Physical Therapist to deliver skilled, patient-centered care in patients' homes across Grand Traverse County and surrounding areas. This full-time role offers autonomy within an interdisciplinary team and a generous bonus program.

You will perform evaluations, develop therapy plans to improve strength and balance, educate patients and caregivers, document in the EMR, and travel to patient homes.

Qualifications

• Active Physical Therapist license in the state you work.
• At least 1 year of PT experience delivering quality patient care.
• Valid driver's license and ability to drive ~20% of workday.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform evaluations, mobility assessments, and create therapy plans to improve strength, balance and independence.
• Educate patients and caregivers on exercises, injury prevention and safe mobility at home.
• Prescribe and document care with thorough EMR records.
• Work independently, but collaborate when needed.
• Travel to patient homes within assigned territory.
