Residential Home Health and Hospice in Livonia, MI is hiring a Physical Therapist (PT) to provide skilled care in patients’ homes across Livonia and nearby areas. A 10,000 first-year success bonus is offered and travel to patient homes is required.

Qualifications

• Active Physical Therapist license in the state you work
• At least 1 year of Physical Therapist experience
• Valid driver’s license and ability to drive ~20% of workday to patient locations

Responsibilities

• Deliver patient-centered skilled care in home health settings with an interdisciplinary team
• Perform comprehensive evaluations and develop therapy plans to improve strength, balance, and independence
• Educate patients and caregivers on exercises, injury prevention, and safe mobility
• Maintain compliant EMR documentation
• Work independently while collaborating with the team
• Travel to patient homes within the assigned territory

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
