Graham Healthcare Group is seeking a licensed Physical Therapist to provide skilled home health services in Michigan’s Southgate/Brownstown area. You will travel to patient homes, assess patients, and design personalized therapy plans to improve mobility and independence.

The role offers autonomy, collaboration with an interdisciplinary team, and a first-year $10,000 bonus for achieving key milestones, with ongoing growth opportunities through our Purdue University partnership.

Qualifications

• Active Physical Therapist license in the state you work (PT).
• One year of experience as a Physical Therapist in a clinical setting.
• Valid driver’s license; ability to drive to patient locations about 20% of the workday.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform comprehensive patient evaluations and develop individualized therapy plans to improve strength, balance, and independence.
• Educate patients and caregivers on exercises, injury prevention, and safe mobility strategies.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
