Job summary

Residential Home Health and Residential Hospice is seeking a Home Health Physical Therapist to provide skilled care to patients in their homes, primarily in Lewistown, PA, with occasional travel to nearby areas.

You will assess mobility, develop and monitor therapy plans, and educate patients and caregivers on exercises and safe mobility. A valid PT license and at least one year of home health experience are required; a driver's license and reliable vehicle are preferred.

Qualifications

• Active Physical Therapist, PT license in the state you work.
• One year of experience as a Physical Therapist.
• Current driver's license and ability to drive to patient locations (~20% daily).

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform comprehensive patient evaluations and develop therapy plans to improve strength, balance, and independence.
• Educate patients and caregivers on therapeutic exercises and safe mobility strategies.
• Ensure compliant EMR documentation and think critically to solve problems.
• Work independently while collaborating with the team.
• Travel to patient homes within assigned territory.

What You'll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence

• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home

• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone

• Travel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Current driver's license and ability to spend ~20% of your day driving to/from patient locations
