You will deliver skilled care, perform evaluations, educate patients and caregivers, and collaborate with an interdisciplinary team.

Qualifications

• Active Physical Therapist (PT) license in the state you work.
• One year of experience as a Physical Therapist.
• Valid driver’s license and ability to drive to patient locations (~20% travel).

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform comprehensive patient evaluations, mobility assessments, and develop therapy plans.
• Educate patients and caregivers on therapeutic exercises and safe mobility.
• Ensure thorough EMR documentation.
• Travel to patient homes within assigned territory.
• Work independently, but with team support.

What You'll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
