Residential Home Health and Residential Hospice is hiring a Home Health Physical Therapist to provide skilled care to patients in their homes in Holt, MI. The role requires a PT license in the state and involves travel to patient homes within the assigned territory.

You will perform comprehensive evaluations, create therapy plans, educate patients and caregivers, and document in EMR. A $10,000 first-year bonus rewards achievement in the initial year, with ongoing rewards thereafter.

Qualifications

• Active Physical Therapist (PT) license in the state you work
• One year of experience as a Physical Therapist
• Valid driver’s license and ability to spend ~20% of time driving to patient locations

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
