Residential Home Health and Residential Hospice in Dewitt, MI is seeking a Home Health Physical Therapist to deliver skilled, patient-centered care in patients' homes. You will perform evaluations and develop therapy plans to improve strength, balance, and independence.

Educate patients and caregivers on exercises and safe mobility, work with an interdisciplinary team, and travel within the assigned territory.

Qualifications

• Active Physical Therapist license in the state you work.
• Minimum one year of physical therapy experience.
• Current driver's license and ability to spend ~20% of your day driving to/from patient locations.

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence.
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.

PTQualifications
