Job summary

Mary Free Bed at Home is hiring a Physical Therapist (PT) for a Home Health position in the Muskegon area. The role offers a $10,000 first-year success bonus and an additional $5,000–$15,000 sign-on bonus depending on experience, with rewards increasing with tenure.

You will deliver skilled, patient-centered care in home health settings, perform evaluations, educate patients and caregivers, and document in EMR. Travel to patient homes is required within the territory.

Qualifications

• Active Physical Therapist license in the state you work.
• At least 1 year of experience delivering patient care as a PT.
• Valid driver's license and ability to spend ~20% of day driving to patient locations.

Responsibilities

• Deliver skilled, patient-centered home health care in patients' homes.
• Perform comprehensive patient evaluations and develop therapy plans.
• Educate patients and caregivers on exercises and safe mobility.
• Ensure compliant EMR documentation and maintain professionalism.

What You'll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
