Job summary

Residential Home Health and Residential Hospice is seeking a Home Health Physical Therapist to provide skilled, patient-centered care in patients’ homes across Charlotte, Eaton Rapids, Dansville, Olivet, Pottersville and surrounding areas.

The role requires a PT license, at least one year of experience, and the ability to travel to patient homes. A $10,000 first-year bonus is offered, with ongoing rewards as you grow with the company.

Qualifications

• Active Physical Therapist license in the state you work.
• One year of experience as a Physical Therapist.
• Valid driver’s license and ability to drive ~20% of your workday.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence.
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, but never alone; travel to patient homes within the assigned territory.

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
