Job summary

Residential Home Health and Hospice is hiring a Physical Therapist (PT) to deliver skilled care in patients' homes across Charlotte and surrounding areas. This position includes a $10,000 first-year success bonus and ongoing opportunities for growth.

You will conduct evaluations, develop therapy plans to improve strength, balance, and independence, educate patients and caregivers on exercises and safe mobility, and complete compliant EMR documentation. Travel is required.

Qualifications

• Active Physical Therapist license in the state you work
• One year of experience as a Physical Therapist
• Valid driver's license and ability to travel ~20% of workday

Responsibilities

• Deliver skilled, patient-centered care in home health with an interdisciplinary team
• Perform comprehensive patient evaluations and develop therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises and safe mobility
• Maintain thorough and compliant EMR documentation
• Work independently while traveling to patient homes within the territory

What You'll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
