Job summary

Residential Home Health and Hospice in Kalkaska, MI is hiring a Physical Therapist to deliver skilled, patient-centered care in patients' homes. This home health role offers autonomy within an interdisciplinary team and meaningful opportunities to impact recovery and mobility.

You will perform comprehensive evaluations, develop individualized plans to improve strength, balance, and independence, educate patients and caregivers on exercises and safety, and document in an EMR.

Qualifications

• Active Physical Therapist license in the state you work.
• One year of experience as a Physical Therapist.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone; travel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
