Job summary

CenterWell Home Health is seeking a licensed Occupational Therapist to assess, plan and deliver home-based rehabilitation aimed at improving daily living skills and mobility. You will collaborate with physicians and the rehab team to create and revise the Plan of Care and document patient progress accurately.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess/screen daily living/work-related skills and develop retraining programs.
• Confer with physician and care team to develop Plan of Care.
• Provide therapeutic treatment to improve strength, coordination, ROM and function.
• Document patient progress and care observations thoroughly.
• Coordinate care and discharge planning as part of the rehab team.
