Job summary

CenterWell Home Health is seeking an Occupational Therapist to support patients at home, reporting to the Home Health Branch Administration. You will assess daily living skills, collaborate on the Plan of Care, and provide rehabilitation to restore strength, mobility and independence.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.
• Minimum six months of occupational therapy experience.
• Home Health experience (preferred).

Responsibilities

• Assess/screen patients' daily living and work-related skills and plan retraining programs with measurable goals.
• Collaborate with physicians and the care team to update the Plan of Care.
• Provide therapy to improve strength, coordination, ROM and function per orders.
• Review and expand retraining programs to meet behavioral goals of each patient.
• Consult with vocational team members to tailor plans to patient needs.
• Report patient status to physicians, rehab staff, and family.
• Participate in care coordination and discharge planning; educate patient and family.
• Design community reintegration activities and coordinate with the inter-disciplinary team.
• Recommend adaptive equipment to improve home/work environments.
• Train staff and family in retraining routines and daily living skills.
• Supervise OT Assistant and Home Health Aide during service delivery.
• Document care observations and interventions daily; submit plans and discharge summaries.
