Job summary

CenterWell Home Health in Prescott, AZ seeks a licensed Occupational Therapist to deliver in-home rehabilitation, restore function, and support patients’ independence through personalized therapy plans. You will assess activities of daily living, design retraining programs, and oversee staff involved in the recovery process.

Responsibilities include collaborating with physicians, documenting patient progress, and contributing to discharge planning to ensure safe, effective outcomes in the home

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop retraining programs.
• Provide therapeutic treatment to improve strength, range of motion and function at home.
• Collaborate with physicians and the care team to update the Plan of Care.
• Document observations, interventions and progress in a timely manner.
• Supervise Occupational Therapy Assistant and Home Health Aide performance.
