Job summary

CenterWell Home Health seeks a licensed Occupational Therapist to deliver in-home rehabilitation, focusing on strengthening, mobility and independence for patients in their homes. You will assess and treat, coordinate with physicians, and document progress to drive better outcomes.

You will collaborate with a multidisciplinary team, participate in care planning and discharge, and help clients regain daily living skills with adaptive strategies and family coaching.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.
• Minimum six months of occupational therapy experience.
• Home Health experience.

Responsibilities

• Assess/screen patient’s daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the patient’s physician and other health care team members and participate in development/revision of the Plan of Care.
• Provide therapeutic treatment and instruction to patients to improve/restore strength, coordination and function.
• Review/expand retraining programs to align with behavioral goals and rehab guidelines.
• Consult with vocational team members to develop rehab plans.
• Document patient status and progress to physician and care management staff.
• Participate in care coordination and discharge planning activities.
• Design community reintegration activities to aid physical reconditioning and adjust plans with the team.
• Design/train staff and family to carry out retraining including activities of daily living.
• Supervise OT Assistant and Aide performance and document care observations and plans.
