Job summary

CenterWell Home Health is hiring an Occupational Therapist to provide patient-centered home health services. You will assess daily living skills, design retraining programs, and work with physicians to optimize the Plan of Care.

You’ll deliver treatment to improve strength, coordination and range of motion, and document patient progress for coordination with the care team. The role requires OT licensure, CPR certification, and a valid driver’s license with reliable transportation.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess daily living/work-related skills and develop retraining programs with measurable objectives.
• Collaborate with physicians and care team to revise the Plan of Care.
• Provide therapy to improve strength, ROM, and function.
• Document patient progress and ensure care plans are implemented.
