Job summary

Qualifications

Responsibilities

• Assess daily living/work skills and develop retraining programs with measurable objectives.
• Administer and interpret functional tests to support physician evaluation.
• Confer with physicians and care team to update the Plan of Care.
• Provide therapy to improve strength, ROM and function per orders.
• Document patient status and progress accurately in day-of-service records.
• Coordinate care and participate in discharge planning with the team.
• Design community reintegration activities and train staff/family.
• Supervise OT Assistant and Home Health Aide performance.
