Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist to lead in-home rehabilitation, assess daily living skills, and design individualized retraining plans to restore function and independence. You will coordinate with physicians and care teams, document progress, and support patient discharge planning.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.
• Minimum six months of occupational therapy experience.
• Preferred: Home Health experience.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the patient's physician and other health care team members and participate in development/revision of the Plan of Care Treatment.
• Provide therapeutic treatment and instruction to patients to improve/restore strength, coordination, range-of-motion and function.
• Review/expand retraining programs in alignment with patient goals and rehab program guidelines.
• Consult with vocational team members to develop and implement vocational plans.
• Report patient status and progress to the physician, rehabilitation staff, Clinical Manager, patient and family members.
• Participate in care coordination and discharge planning activities and act as a resource to other health care personnel.
• Design community reintegration activities to assist the client in reconditioning and coordination with the interdisciplinary team.
• Recommend/adapt equipment to improve residence or work environments and participation in rehab.
• Design/train staff and family in retraining programs and activities of daily living.
• Supervise and evaluate OTA and Home Health Aide performance.
