Qualifications

• Active Occupational Therapy licensure in the state of practice.
• Valid driver's license; access to a dependable vehicle or public transit.
• Ability to work within home health settings and coordinate with a care team.

Responsibilities

• Provide OT services within scope of practice per plan of care.
• Evaluate functional status and CMS-required documentation with physicians.
• Record responses to treatment and note changes to patient condition.
• Conduct patient assessments of function using standard tests.
• Function effectively in diverse home environments.
