Job summary

UnitedHealth Group is seeking an Occupational Therapist for Home Health to assess and evaluate patient needs related to functional status, activities of daily living, fine motor skills, home safety, and adaptive equipment. The role emphasizes evidence-based practice and coordinated care within the health team.

Primary responsibilities include evaluating functional status, developing therapy plans with physicians, and documenting patient progress.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy, coordinating with the care plan.
• Evaluates functional status and CMS documentation requirements, consults with physician on therapy plan.
• Observes, records, and reports patient responses and changes to condition to nursing supervisor and physician.
• Conducts patient assessments using functional ability tests.
