Job summary

CenterWell Home Health is seeking an Occupational Therapist to deliver in-home rehabilitation, helping patients regain strength, mobility and independence in the comfort of their own homes.

You will assess, plan and implement therapy, collaborate with physicians and the care team to update the Plan of Care, and document progress and outcomes to support ongoing recovery and safe discharge.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.
• Minimum six months of occupational therapy experience (preferred).
• Home Health experience (preferred).

Responsibilities

• Assess/screen patients' daily living/work-related skills and develop retraining programs.
• Administer and interpret functional tests in patients' homes to guide care.
• Collaborate with physicians and the care team to update the Plan of Care.
• Provide therapeutic interventions to improve strength, coordination, ROM and function.
• Document patient progress and care observations accurately on a timely basis.
• Coordinate care and discharge planning and support family education.
