Job summary

CenterWell Home Health in Pullman, WA is seeking an Occupational Therapist to assess and treat patients in home settings, collaborating with physicians and care teams to restore function and independence. You will design retraining programs, coordinate discharge planning and support adaptive strategies for daily living activities.

The role requires current OT licensure, CPR certification, and a valid driver’s license with transportation.

Qualifications

Current and unrestricted OT licensure.
Current CPR certification.
Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

Assess/screen patient’s daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
Confer with the patient’s physician and participate in Plan of Care development and revision.
Provide therapeutic treatment and instruction to improve strength, coordination and range of motion.
Review/revise retraining programs to align with behavioral goals of the rehab plan.
Design community reintegration activities to assist the client in physical reconditioning and coordination with the interdisciplinary team.
Document patient care observations, interventions and evaluations accurately and timely.
