Job summary

CenterWell Home Health in Pullman, Washington is seeking a licensed Occupational Therapist to deliver in‑home rehabilitation services and support patient independence.

You will assess daily living activities, design retraining programs, and collaborate with physicians to optimize recovery, while documenting care plans and progress. Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Assess and screen patient’s daily living/work-related skills and develop retraining programs.
• Consult with physicians and rehab team to update the plan of care.
• Provide in‑home therapeutic interventions to improve strength, ROM, coordination.
• Review and expand retraining programs to align with goals and rehab plan.
• Coordinate with vocational team to support patient needs.
• Document patient progress and plan of care with supervisor.
• Participate in discharge planning and care coordination.
• Design community reintegration activities to support home and daily living.
• Educate staff and family on retraining programs and activities.
